Equipment Hanout - Sat July 17th 2010
Parents,
The equipment handout is scheduled for Saturday July 17th from 8 AM to Noon. The following documents must be turned in, to be eligible to receive any equipment:
1). Original birth certificate (will be returned imemdiately) and 1 copy
2). Original final report card for the 2009 - 2010 school year (will be returned immediately) and 3 copies. Copies must be 8 1/2 x 11 and have all comments visible.
3). 2010 Physical Form. We will have a physician present to conduct physicals at a cost of $10 each. If you choose to your use own physician, you must use the Pop Warner form. No other physical forms can be accepted.
4). Little Scholars Pre-Application Form
All of these forms should have been emailed to you by your coach. If you do not have these forms, please contact your coach.
The scheduled report times are as follows:
(1). A – C 8:00 – 8:30 AM
(2). D – F 8:30 – 9:00 AM
(3). G – K 9:00 – 9:30 AM
(4). L – M 9:30 – 10:00 AM
(5). N – Q 10:00 – 10:30 AM
(6). R – T 10:30 – 11:00 AM
(7). U – Z 11:00 – 11:30 PM
For those participants that miss the equipment handout, you will need to report to the field early the first day of practice which is Monday August 2nd. More info to come later.
